WHAT DO YOU NEED TO QUALIFY?

• You are at least 18 years of age
• You have an open and active checking or savings account
• You have been employed for at least 90 days
• You have a picture ID
• You have access to a fax machine
• You must be able to receive a fax from us

IF YOU QUALIFY

We have provided two convenience ways to apply! You can either download the application or fill it out online and submit it electronically. If you choose to download, neatly complete the application in black ink.

WE WILL BE UNABLE TO PROCESS AN INCOMPLETE APPLICATION

If you choose to download, then you will need to fax us the completed application at 1.888.545.8972.

Whether you decide to download or apply online you will also need to fax to us at, 1.888.545.8972:

• Your most recent Checking account bank statement (must be within 30 days)
• or Your most recent Savings account statement (for the past 90 days)
• Your most recent pay stub
• A voided personal check or savings account card w/ routing number
• A copy of your driver's license or picture ID

WHAT WE WILL DO

We will review your application, and call you if we have questions.

We will FAX you an Agreement that must be signed and returned to us promptly.

We will ACH (deposit) the agreed upon amount into your account. If we receive your Signed Agreement by 3:00 P.M. EST, the funds will be in your account the next business day. If we receive your Agreement after 3:00 P.M. EST, the funds will be available two business days from today's date.

HOLIDAYS WILL DELAY ACH DEPOSITS BY ONE DAY
